So I was just looking at my unicorn Buttercup and noticed that she has 5 children but it says that she has only been bred 3/21 times, and I was wondering how that was possible?

There are item/s that make pets have twins. It looks like that item was used several times on her. It was from the warlock portal event a while ago.

But most of the children she became pregnant with and had after I bought her and I never put an item on her? I;m pretty sure she only had one child before I bought her. She never even had any twins while I owned her either, so that's why I was confused.

Perhaps while exploring on an island you encountered a shrine which got your pet pregnant? She has a child but that child doesn't count towards your pet limit.

Yes, if you encounter the fertility shrine and say "yes" your pet will have a child but won't count against them. Liandralithe is correct :)
Monues is also correct- but you would need an item, or a pet with fertility (symurgh). :)
